I know a few people that are having fertility issues without a i so it's not uncommon right now. Patience will be the key. What sort of feed are you using? Fertility will kick in.

It looks like you are on the right track. I agree - be thorough with your cleaning. Oxine can also be used for fumigation. I also do a tylan dip of all eggs prior to incubation in order to reduce disease and myco in my flock.
I also incubate bantam vs. standard eggs differently, using dry incubation for first 18 days in my bantams and 45-50% humidity for standards. If I have to incubate both types together then I lean towards lower or dry humidity range. I use multiple thermometers at different levels - even with fans you will get pockets of temperature differences. If you can rotate/turn your egg trays to different positions to minimize effects from air pockets.
Also, avoid opening up the hatcher as much as possible - leave the fuzzies in there until you absolutely need to empty them out (they are good with their egg sac for food for a long time!) or you will affect the other eggs pipping and the chicks working their way out will not be able to pip correctly.
And yes, switch to a breeding diet a month prior to collecting eggs for hatching.

Word of caution regarding Oxine:
I've been having a very deep discussion with a very trusted friend on Oxine. Consensus. Do not use it. Warning it is very dangerous and the only way it actually works is to activate it and when you do actually activate it it creates chlorine gas.....
Virkon and tektrol are very good disinfectants and I even know some people using natural products.
